Direct credit form - have your claims paid into a bank account

Direct credit provides an easy way for us to pay your claims straight into a bank account. Setting up direct credit on your policy means that you don't have the hassle of having to pay a cheque into the bank and then waiting for it to clear. Also you don't have to worry about cheques getting lost or delayed in the post.

If you would like your claims to be paid by direct credit in the future simply download and complete a direct credit form and return it to:

If you have a go-active policy as part of your company benefits package, this is the form to download if you want to make a claim. If you're unsure if you have a go-active policy, please check your policy certificate as it will say "go-active" on it.
